Why is it so hard to lose weight for some people?

i don't understand people...it's pretty straightforward - exercise and eat healthy...drink lots of water, eat light foods, eat less. i hear some people going like "oh i wish i could lose a couple more of these pounds"...WHILE they're eating mcdonalds or a chocolate bar. i understand if you have a disease, but honestly
